Logisticare has new requirements for all drivers who want to participate in their program. Signup period has been extended to Friday, September 13th.
Drivers are encuraged to complete the following requirements as soon as possible and avoid a last minute rush:
- Copy of Driver’s License
- Original DMV printout (K4 3 year) $10
- Original DOT Medical Examination Report (MCSA-5875) $110.50
- Copy of DOT Medical Examination Certificate (MCSA-5876)
- Copy of DOT Regulatory Drug Test Results $83.50
- Original DOT Alcohol Test Results $45.50
- Copy of valid CPR Certificate (Prices vary at different locations)– All documents must be submitted to Cynthia by Friday September 13th to continue to qualify for rides. For Medical Exam, Drug and Alcohol go to: Concentra (any location in Bay Area is ok)
