Paratransit slips must now include your personal drivers license number. All fields must be filled out for payment.
Add your California Drivers License number (CDL) in the box that says Driver ID number. Add your name and driver ID number in the box above it.
Reminder: Paratransit slips are necessary only when you are unable to use the tablet.
